DIABETES EDUCATION & MANAGEMENT
Work one-to-one with our pharmacist and Certified Diabetes Educator at Leeds Pharmacy.
Learn what each medication does, how to use it, what side effects to discuss and how it fits into your daily routine. We review prescription medicines, non-prescription products and supplements together.
Review glucose records and available A1C results, discuss patterns and understand the goals agreed with your care team. Learn when readings need further attention and what information to share with your clinician.
Get hands-on education about glucose meters, monitoring devices and injection technique, including insulin pens or other prescribed injectable treatments. Bring the device you use so we can review your technique.
Learn to recognize signs of low or high glucose and understand the response plan appropriate for you. We can help you identify questions to discuss with your prescriber and situations requiring prompt assessment.
Discuss how meals, activity, work, travel and changing routines affect medication use and monitoring. The aim is a realistic routine you can follow, with manageable next steps.
Follow-up sessions can revisit your questions, technique and medication routine as treatment or circumstances change. We work with your prescriber when medication-related concerns need coordination.
We begin with your priorities, review the information you bring, and agree on practical education goals. You can ask about a newly diagnosed condition, a new device or medication, or a routine that has become difficult to manage.
MedsCheck for Diabetes is available to eligible patients. Our team confirms program eligibility and explains any applicable fees before the service.
Your health card, medication containers or current list, glucose records, relevant laboratory results if available, and any meter, monitor or injection device you want to discuss.
